The only province specifically required to for the Roman Empire is the province of Rome, the other 424 provinces can be from anywhere else shown above. This means that it is, for example, possible to form the Roman Empire while owning only 7 provinces in Italy (as 50 provinces can be skipped and Italy has 57), or by skipping Anatolia entirely.

She is not a candidate for the Holy Roman Empire, so she'll need to take enough territory to form Italia and win independence before she can make an empire title. She'll also need to take the Unify Italy decision to unlock the Restore the Roman Empire decision.
Jul 15, 2020 · Thanks - little tip for others not starting as Byzantine: If as a Catholic with Holy Fury you create the Latin Empire (which I did following Venice making the first two crusades sacks on Greece and Thrace respectively), you cannot restore the Roman Empire. The 800 prestige doesn't seem worth it now.
May 15, 2019 · Tiberius did not undertake any "reign of terror" (i.e. treason trials) until AFTER Germanicus died. Tiberius didn't wear a breastplate all the time. Germanicus died in Antioch, not Germania. Shots of the Roman forum included the arch of Septimius Severus, which wasn't built until nearly 200 years later.
Not gonna spoil the fun stuff about it but it makes a Roman Empire run really fun when you start there, but you do need to pick up a few duchies in the Mediterranean, like Sicily, sardinia, Corsica and the islands closest to Iberian peninsula. You only need 2 of the duchies to unlock the secret decision for the kingdom.

The Eastern Roman Empire became a thing the minute theodosius gave half of the empire to Arcadius. You should be able to unlock this decision as soon as you’ve recovered the 476 borders, (the Eastern half of the Roman Empire) and Justinian’s Ambition should turn you into the Roman Empire, or dissolve the Holy Roman Empire
May 8, 2024 · This is not the fastest Roman Empire, but it's reliable and doesn't rely on any paratrooper cheese. Bring your entire army to Ethiopia, you'll want mountaineers in the north and tanks/cavalry in the south.
To restore the Roman Empire in CK2 you only need ~50ish provinces (14 duchies and being Byzantium emperor) hence the nice CB. If should be doable to create a mod to proclaim to be Rome if you have enough province (plus some keys provinces) and have some gov mechanics about your legitimacy has Roman Empire...
