This document contains two account closure forms for the Atal Pension Yojana (APY) pension plan - one for voluntary exit and one for closure due to death of the subscriber. The voluntary exit form allows subscribers to close their account before age 60 and receive refunds of their contributions.
The branches of APY-SP shall collect the APY Account Closure Form from the Subscriber/spouse/nominee/claimant as per the format stipulated by Pension Fund Regulatory and Development Authority (PFRDA).
