Read means the user that you sent the message to actually opened up the iMessage app. If it says delivered, they most likely didn't look at the message although it was sent through. You can change it though in Settings to make it that you don't send a read receipt when you read a message.

Unlike “Delivered,” the “Read” status indicates that the recipient has opened or accessed your message. It provides you with the assurance that your message has been acknowledged and seen by the recipient. Email and messaging platforms determine the “Read” status using various techniques.
When read receipts are turned on, the sender sees the word “Read” below messages they’ve sent to you, indicating that you’ve read their message. If you haven’t read their message, the sender sees “Delivered.” You can turn on read receipts for everyone or for individual contacts.
