To register as new elector, you can apply in Form 6 and for replacement of EPIC(existing elector) you can apply in Form-8 either offline through BLO(Booth Level Officer)of your area or online through voter service portal (at https://voters.eci.gov.in/) or through Voter Helpline Mobile App to fill the form.
