They both have the same meaning, as in 'be quiet', except 'shut the hell up' would be considered more of an offensive phrase, since the word 'heck' is to replace the word 'hell'. Don't ask me why. It's just like that. Also, 'shut the hell up' could be aggresive depending on the tone.

Uskut - means be quiet. ikhras - shut up. battal el kalam - means 'stop the talking'. If you wanted to be polite and you were in a gathering you could say, 'lo sma7t, sukoot!' = 'please, quietness!' ('a bit of quiet please!')

She was spitting abuse at me and my family, so I gave her a shut up call. The meaning in this context is to react in an insulting or aggressive way to make the other person stop talking.

You've introduced an interesting distinction with the scalpel question. Hand me the scalpel, will you! This carries the sense of irritation I've mentioned above.

Something like "Shut up, will you!" is just a rearrangement of "Will you shut up!" that we use when we want to "edit" an imperative after the fact to something more like a question.
