In other words the entire things means: "To change this document or to say it has been (so) changed is a serious offence". or perhaps. "To change this document or to suggest / represent that it has been changed is a serious offence".

The other two are examples of alteration. Any attempt to alter the document voids it, but doesn't necessarily make it a forgery in the same sense.
