After watching it, I finally read up on a few articles about the production of the movie. Man, what a f'ed up story-line behind this one. It took about 15 years for this one to be released due to inexperience, meth addiction, meticulous editing, and THEN the director passes away before it's released.

As a 16 year old the grandson J Paul Getty the III was kidnapped by Italian mob gangsters. The rich old man, the grandfather, refused to pay the ransom. They cut off the boy's ear and sent it to him. Still the old man wouldn't pay, till it went down to 3 million. The boy's kidnapping ordeal lasted five months.
