Karoline "Kara" Spencer (Elisabeth Harnois) is the main protagonist and plot-twist main villainess from the 2018 Lifetime film, Psycho Ex-Girlfriend (alternately titled, Twisted; airdate May 19, 2018).
Kara Spencer is a therapist who also works as a waitress at a restaurant alongside her husband, Tyler; though in the film's opening scenes, she is visited by a woman named Elle Lloyd, who entered after Kara's session with the regular patient Laura Fischer. After Elle and her police chief father appeared as customers at the restaurant, Kara learns that Elle is Tyler's ex-girlfriend, with the relationship taking place during a time where both Tyler and Elle were addicted to drugs.
The film's steady progression sees Kara on the receiving end of apparent harassment from Elle, with incidents including posting newspaper clippings from the tragic boating accident that killed Kara's parents when Kara was just 13 and vandalizing Kara's wedding dress. Kara was later shocked to see her cat mutilated, with suspicions that Elle was behind the heinous action. Kara and Tyler's attempts to report Elle to police fell short, with Chief Lloyd being defensive when it came to his daughter and being accused of covering for her.
Later on, Kara was on the receiving end of an attack by a masked man in her home, after which she and Tyler went to Detective Clemens with their accusations against Elle. After a second man attacked Kara, Clemens appeared and arrested the attacker, and got to work investigating Elle, leading to a restraining order being filed against her. Elle, who had denied being behind everything that occurred, appeared at Kara's house in the film's climax and angrily confronted Kara, whose attempts to keep Elle out of the house were thwarted when Elle smashed her way inside with an ax and later pursued Kara. At that moment, Kara took out a gun and held Elle at bay, only for Tyler to enter and convince both women to lower their weapons, only for Kara to shoot and kill Elle once she moved towards her.
The film's final scenes showed a pregnant Kara being informed by Tyler that he would be meeting with Chief Lloyd, with Kara stating that Lloyd couldn't come to terms with what happened to his daughter before going to visit Laura and her daughter. After asking Laura if she had heard about Elle, Kara suddenly turned heel by revealing that she had set up Elle, with Laura uncovered as Kara's accomplice in the scheme. Laura was revealed as the one who set up the rape fantasy ad, adding that she had taken it down in response to Kara's question about other men contacting her. Kara's heel turn also indicated/revealed that she had set up all of the other incidents, including killing her cat, as part of her plan to eliminate Elle.
