When I started this game and downloaded the patch to finally "unlock" the features, I at first had difficulty trying to raise the affections of the women or even get any specific scenes. That's why my playthrough the first time was a total flop. But after I troubleshoot my problems, it turns out that the game operates depending if you have the DLC. So for those that are new and had difficulty, here is how to resolve this so you degenerates can get FAP material.

If you have max affection with her, then you unlock the threesome ability. Then raise Miyuki's affection to max, have a threesome, raise her alarm level to max, unlock foursome.
